Overview

Form 3D is used to notify MAS that a provisional representative has fulfilled the outstanding CMFAS examination requirements within the stipulated grace period. Filing Form 3D converts a provisional representative to full appointed status, allowing them to continue conducting regulated activities without time restrictions.

When to File Form 3D

Trigger Conditions

  • A representative was appointed provisionally via Form 3B
  • The representative has passed all required CMFAS examination modules for their declared regulated activity
  • The examination results are confirmed and verifiable through the Institute of Banking and Finance (IBF) records

Filing Deadline

  • Must be filed within the 3-month grace period granted under the provisional appointment
  • The grace period starts from the date of provisional appointment as recorded by MAS
  • Late filing may result in automatic cessation of the provisional appointment

Consequences of Not Filing

  • If Form 3D is not filed within 3 months, the provisional appointment lapses automatically
  • The representative must cease conducting all regulated activities immediately upon lapse
  • A new Form 3A (full appointment) must be filed if the individual wishes to be re-appointed
  • The principal must notify affected clients of the cessation

Conversion from Provisional to Appointed Status

Pre-Filing Requirements

  1. Examination Completion: All outstanding CMFAS modules must be passed
  2. IBF Records Updated: Examination results must be reflected in IBF's database
  3. Fit and Proper Assessment: The principal must confirm the representative continues to meet Fit and Proper criteria
  4. No Regulatory Actions: No pending enforcement or disciplinary actions against the representative

Step-by-Step Conversion Process

  1. Representative passes remaining CMFAS exams
  2. Obtain official examination results from IBF
  3. Ensure all modules required for the declared regulated activity are covered

  4. Principal verifies examination results

  5. Cross-reference IBF records with required modules
  6. Confirm certificate numbers and examination dates

  7. Complete Form 3D

  8. Particulars of the representative (name, NRIC, date of birth)
  9. Particulars of the principal (CMS/FA licence number)
  10. Original provisional appointment reference number
  11. CMFAS examination results (module, date passed, certificate number)
  12. Declaration that all required examinations have been fulfilled

  13. Submit via MAS CoRe system

  14. Log in to the Corporations and Representatives (CoRe) system
  15. Select "Lodge Form 3D — Examination Fulfillment"
  16. Upload supporting documents (examination certificates)
  17. Submit with authorized signatory approval

  18. MAS processing

  19. MAS reviews the submission within 14 business days
  20. If approved, the representative's status changes from "Provisional" to "Appointed" in the public register
  21. The appointment becomes ongoing with no time restriction

Section 1: Representative Particulars

  • Full legal name (as per NRIC/passport)
  • NRIC number or passport number
  • Date of birth
  • Current representative registration number

Section 2: Principal Particulars

  • Name of CMS licensee or Licensed Financial Adviser
  • Licence number
  • Contact person for this submission

Section 3: Original Provisional Appointment Details

  • Date of provisional appointment (Form 3B lodgement date)
  • Form 3B reference number
  • Regulated activities declared under provisional appointment

Section 4: Examination Fulfillment Details

  • CMFAS module(s) passed
  • Date(s) of examination
  • Certificate number(s)
  • Confirmation that all modules required for declared activities are now complete

Section 5: Declaration and Undertaking

  • The principal confirms the information is accurate
  • The principal confirms the representative continues to satisfy Fit and Proper criteria
  • Authorized signatory (CEO, director, or compliance officer) signs off

Fees

  • No additional lodgement fee for Form 3D
  • The original lodgement fee of $200 paid with Form 3B covers the conversion
  • Annual representative fees continue to apply at the existing rate ($200 or $700 depending on activity type)

Common Issues and Pitfalls

Timing Issues

  • Filing too close to the 3-month deadline leaves no buffer for MAS processing
  • Recommended to file at least 2 weeks before the grace period expires
  • If examination results are delayed by IBF, contact MAS proactively to request an extension

Documentation Gaps

  • Ensure examination certificates match the NRIC/passport number on the original Form 3B
  • Missing or mismatched certificate numbers will cause rejection
  • All modules for the declared activity must be covered — partial fulfillment is not accepted

Status During Processing

  • The representative retains provisional status while Form 3D is being processed
  • If the 3-month period expires while Form 3D is under review, MAS generally allows continued activity pending the outcome
  • The principal should document the pending status for audit purposes
